CME Evaluation Instructions

To expedite the distribution of your continuing medical education credits, please read the following very carefully.

NOTE: CME evaluations can only be submitted electronically through FICS Online. You may access the evaluation forms for specific sessions by using the meeting APP (www.ficsonline.org/SA23). Once you navigate to the APP, click on the “Evaluation Forms” link.   If you prefer, you may also navigate directly to the evaluation form page of FICS Online by going to www.ficsonline.org/eval23.

Continuing Medical Education Transcripts will be distributed via email no later than 6 weeks after the conference ends, but only after you submit your post meeting survey.  The post meeting survey will be available in approximately 4 weeks after the conclusion of the meeting. Only paid registrants will receive CME credit. Invited non-member speakers are eligible to receive CME credit for the session in which they are speaking without payment of registration fees, but an evaluation form must be submitted.

In order to accurately complete your evaluation forms, please read the following carefully. Doing so will allow us to more effectively evaluate the program and also meet ACCME requirements.

Presentation Rating
For each session that you attend, rate the moderators as well as each presenter/presentation in the session by clicking the appropriate radio button. At the end of the presenter evaluation section you must also indicate whether or not you detected any commercial bias in the presentations you observed. If you believe commercial bias was present, you must explain in detail, including the name of the speaker and the nature of the bias. Please note that referring to a specific product or briefly identifying a manufacturer does not in and of itself determine commercial bias. Commercial bias is defined as an attempt by the presenter to promote (flagrantly) a specific product or manufacturer without indication that other options may be available.  

If you do not explain the nature of the commercial bias that you identified, your identification of bias will be considered invalid when the final analysis of the program, session and each speaker is conducted.

There are also a number of questions pertaining to the educational value of the overall session at the end of each form. Your response to these questions is required.

Statement of Completion
Please indicate your hours of attendance in the appropriate field for each session that you attended. You should ONLY include the number of hours you were ACTUALLY in a session. Read the instructions on the individual CME Evaluation Form carefully as specific guidance is provided. Be sure to complete all required fields before clicking submit or your submission will be rejected. Required fields include, but are not limited to your name, the submission date and the number of credits you are claiming. ICSUS reserves the right to adjust credits issued based on our determination of the actual maximum number of credits.

Remember to complete all the forms you submit with as much detail as possible. We prefer if you only submit a form once for each session you attend.

CME Evaluation Forms must be submitted no later than July 1, 2023.
